Understanding Chronic Condition Management Plans

What is a Chronic Condition Management Plan (CCMP)?

As of July 1, 2025, the previous GP Management Plans (GPMPs) and Team Care Arrangements (TCAs) have been streamlined into the new Chronic Condition Management Plan (CCMP). This plan is designed for individuals with a chronic medical condition that has been (or is likely to be) present for six months or longer, such as arthritis, diabetes, asthma, or heart disease.

A CCMP helps your GP coordinate your care by:

  • Identifying your health and care needs.

  • Outlining the services your GP will provide.

  • Listing actions you can take to manage your condition.

Essentially, it creates a structured approach to your ongoing care, especially if you require treatment from multiple healthcare providers.

How can a CCMP help me access physiotherapy?

If your GP creates a CCMP for you, and they determine that physiotherapy is directly related to managing your chronic condition, you may be eligible for Medicare rebates for up to five individual allied health services per calendar year. These five services can be all physiotherapy, or a combination of different allied health services. At MoveWise Home Physio, we offer these visits bulk billed, with no out of pocket fee.

How do I get a referral from my GP?

To access these subsidised services, you'll need a referral from your GP. It's important to discuss your chronic condition and care needs with your GP. They will assess your eligibility and determine if a CCMP is appropriate for you. Your GP will then provide you with a referral letter for allied health services, including physiotherapy, as part of your CCMP.

Key things to remember:

  • Talk to your GP: The first step is to discuss your condition with your doctor and see if a CCMP is right for you.

  • Referral letters: Your GP will provide a referral letter directly to allied health professionals, similar to referrals for medical specialists.

  • Five services per year: You're eligible for up to five subsidised allied health services per calendar year under a CCMP. Unused services can carry over to the next calendar year but will count towards that year's five-service limit.

  • Regular reviews: Your CCMP should be prepared or reviewed by your GP within the last 18 months to continue accessing allied health services. Plans can be reviewed every 3-6 months.
